If you’re suffering from a serious illness, our Palliative Care will work to provide an additional layer of support for both you and your family. Our team can help by working directly with your providers, offering ongoing care, symptom management, advance care planning, counseling, family support and more.
Your wishes and preferences are central to your palliative care. We provide care wherever you may be, whether it's at home, in the clinic, in the hospital or an assisted living facility. Our team includes medical director, advanced practice practitioners, social work, and chaplain.

Our palliative care team can help by:
- Assisting with Advance Care Planning: We can help do things like establish a living will or medical powers of attorney, ensuring your wishes are known if you become unable to speak for yourself
- Defining Goals of Care and symptom management: We will work to help reduce and control pain and debilitating symptoms, and collaborate with your doctor to ensure the best possible treatment. This includes defining care options for you through education and symptom management.
- Support for families: Social work available to provide counseling and connections to various community resources. We also work coordinating family meetings to assist with medical decisions.
- Providing spiritual counseling: Our Board Certified Chaplain provides spiritual support for patients and families from all spiritual and religious backgrounds.
