Care management has always been open to employees with St. Peter’s Health primary care. However, there has never been a specific program for SPH employees, their spouses or dependents to access care management services. Thanks to a CEO Connect from an employee who was struggling to navigate the health care system after their child had a new diagnosis, that is all about to change.
What is a care manager and why would I want to use this service?
A care manager’s role is to help empower a patient’s ability to manage their condition and navigate the health care system. They share the burden of a recent diagnoses and help educate patients so they can effectively manage a chronic condition. They help streamline the process for patients, guiding them through the hoops to receiving care. This includes coordinating with insurance companies to navigate benefits, connecting patients to community resources, coordinating care with provider care teams at St. Peter’s Health or beyond, partnering with pharmacists to review medications and creating a care plan so they are working in tandem with the patient towards a common goal. The duration of this support can be brief or go on for an extended period of time depending on the specific needs of the patient.
Who is our Employee Wellness Care Manager?
We recently hired McCall Elverum to step into this brand new role of the Employee Wellness Care Manager. McCall Elverum, RN earned her Bachelor of Science in Nursing from Creighton University in 2010. She was born and raised in southeastern Idaho but has worked across the country as an RN in various roles. She started at St. Peter’s Health on the Medical Floor in 2014 and transitioned to St. Peter’s Health Medical Group in 2017 to work as an RN Care Manager.
Is the Employee Wellness Care Manager the same as the Employee Health Nurse Coordinator?
The Employee Wellness Care Manager Program lives within the Population Health Department’s Wellness Team, an intentional separation from our Human Resources department. The work that McCall is doing with employees, spouses or dependents is secure; individual information will not be shared with anyone including her direct Manager. For her to engage with participants, they will sign a participation agreement that clearly outlines the roles and responsibilities of the participant and the Employee Wellness Care Manager.
