At St. Peter's Health, we strive to provide the best experience possible for our patients. If you have recently had an experience with our care teams, we understand you may wish to share compliments, concerns or suggestions for improvement regarding you or your loved one's care. This information is valuable for our organization and is vital to our continuous improvements regarding the quality of care, patient safety and overall service to our community.
If you are filing a complaint on behalf of another person and they would like you to speak on their behalf, we require a release of information form in order for us to discuss any patient information. The release of information form can be found here.

If your issue is still unresolved, proceed to step 2.
